Service Outline
We are a team of specialist children’s community nurses who care for children and their families within their community, who have long term complex health needs, often palliative. We as nurses provide clinical assessments, treatments and support. We also educate and train families and carers on specific subjects including diagnosis and clinical procedures.
We are all children's trained nurses with a variety of additional training. We have a skill mix within the team (we all hold a core case load of children with complex health needs) however each nurse has her own specialist role for example oncology, respiratory, rheumatology, cardiology, endocrinology and complex bowel and bladder conditions.
We offer specialist advice and support to young people and their family. Continuity of care for their time within children's services and transitions into adult services.
We provide training to family and health professionals and liaise with multiagency teams around the child.
We also provide a 24 hour on call service every day of the year for young people within the service.
The team is based at The Den but we work in a variety of community settings across North Derbyshire including:
- Mainstream schools and nurseries
- Special school settings
- Families homes
- Link carers
- Short break provision
- Or any other community settings depending on the child and their family’s needs.
We work in collaboration with the young person, the family and the professional's involved in that child's care.
Children’s Community Nurses see children between the ages of 0-19 years old and their families who fit the criteria of complex health needs and are registered with a North Derbyshire GP (excluding Glossop) who are normally living in North Derbyshire.
We accept referrals from families, GP’s and other health professionals as long as the child's legal guardian has given permission/consent.
