The Home Based Support Service is provided in partnership between Chesterfield Royal Hospital NHS Foundation Trust and Fairplay - and is funded by the Clinical Commissioning Groups. The service provides short break care for children and young people with complex health needs that may be life – limiting or life – threatening in North Derbyshire. Care can be provided in the home setting or at the Fairplay Centre, where a session is held once per month on a Saturday, with the aim of providing a multitude of fun activities in a safe and caring environment. The Care Workers are fully trained to perform required medical tasks including feeding via gastrostomy devices, tracheostomy care and administration of medications.

We also invite brothers and sisters over 5 years of age of any member of the Home Based Support Service to join our sibling's group, which provides a range of activities and exciting outings.

Referrals will be considered from any professional or from families directly, and eligibility will be decided by the Nurse Coordinator.
