Our New Trust Strategy.jpgYour Trust: Improving Care Together

We are here to care for the communities of Chesterfield and North Derbyshire. Every day, our colleagues and volunteers provide compassionate, professional care to hundreds of thousands of people, often in challenging circumstances.

Our new strategy sets out how we will build on what already makes #TeamCRH special, while responding honestly to the pressures we face. It focuses on three connected priorities that will guide us over the next five years:

  • Building Strong Foundations - creating the conditions for safe, timely care by investing in colleague wellbeing, modern systems and a compassionate culture.
  • Neighbourhood Health - working with partners to deliver more care closer to home and prevent people from becoming unwell.
  • Tackling Health Inequalities - ensuring fair access, better outcomes and inclusive care for every community we serve.

This strategy is about making everyday care safer, kinder and more reliable - for patients, for colleagues and for our communities. We’ll do this by working together and focusing on what matters most.

Frequently asked questions about our new strategy:

Improving Care Together is our five‑year strategy for our Trust.

It explains:

  • How we plan to improve care over the next five years
  • How we’ll support our staff
  • How we’ll work with partners to keep people well
  • How we’ll make care fairer for everyone

At its heart, it’s about making everyday care safer, kinder and more reliable.

Healthcare needs are changing. More people need care, often with more complex needs, and NHS services are under pressure.

This strategy:

  • Is honest about the challenges we face
  • Gives clear direction at a difficult time
  • Focuses on what will make the biggest difference for local people

It helps us plan for the future while continuing to deliver care today.

There are no sudden changes to services as a result of the strategy.

Over time, you may see:

  • Better coordination between hospital, community and GP services
  • More support to help people stay well at home
  • Easier access and smoother transitions between services

Any changes will be planned carefully and shared clearly.

We have three priorities:

Building Strong Foundations

We’re investing in:

  • Staff wellbeing and leadership
  • Compassionate, respectful culture
  • Modern systems and facilities

This matters because supported staff provide safer, better care.

Neighbourhood Health

We’re working with partners to:

  • Provide more care closer to home
  • Improve prevention and early support
  • Reduce unnecessary hospital visits

This helps people get the right care, in the right place, at the right time.

Tackling Health Inequalities

We want:

  • Fair access to care
  • Inclusive services
  • Better outcomes for all communities

This means recognising that not everyone experiences health in the same way and acting on it.

The strategy focuses on:

  • Listening to feedback
  • Improving everyday care, not just big projects
  • Making care more joined‑up and easier to navigate

Patient experience, feedback and outcomes will be central to how we measure success.

The strategy reflects insight from:

  • Patient feedback, complaints and compliments
  • Engagement with local partners
  • Conversations with staff and volunteers

It builds on what we’ve heard matters most: kindness, communication, access and respect.

We will track progress through:

  • Patient experience and feedback
  • Quality and safety measures
  • Staff survey results
  • Financial sustainability

Progress will be monitored by our Board of Directors and shared openly.
