What is Patient Hub?

Patient Hub is a secure online portal which allows you to access your appointment information and confirm your hospital appointments in one place, by using your smartphone, tablet or computer. It allows you to:

  • Access details about your upcoming appointments
  • Access hospital appointment letters
  • Receive text and/or email reminders about your upcoming appointments 
  • Complete Pre Appointment surveys where required
  • Complete Health-based questionnaires - For those patients on Patient Initiated Follow Up (PIFU) pathways

Chesterfield Royal Hospital is offering more communication choice to patients. Additionally, your hospital is working hard to save costs, such as post and print. These savings mean more money for nursing services and lifesaving equipment, and also improves our carbon footprint.

The reminder service keeps you informed seven and two days before your appointment, meaning you won’t forget or miss important appointments.

You’re also less likely to miss appointments because of postal delays or errors.

Frequently asked questions

No, the hospital will create the account for you. As soon as you are a patient of the hospital, there will be an account in your name. You only need to access it when you need to see your appointment details, and you will receive a text or email informing you of this. Please make sure we have your up-to-date contact details. Without the correct mobile phone number or email, you won’t be able to access Patient Hub. To update your contact details please contact Chesterfield Royal Hospital’s central booking team on 01246 516090 or by email crhft.outpatient@nhs.net or crhft.patienthub@nhs.net.

All patients with a registered mobile phone number or email address will automatically have access to this service. A text message, email or push notification (if you have the NHS App) will be sent. Please then follow these steps:

1. Click on the link.

2. You’ll be asked to enter either your mobile number or email address, together with your date of birth.

3. You will then be sent a 6-digit authentication code by SMS, or by email if we don’t have your mobile number.

4. Enter the 6-digit code. Please be aware the authentication code will expire in one minute

5. Then you are securely logged in to Patient Hub.

Patient Hub has clear login instructions for you to follow. After that, you can go back at any time.

If you do not login into Patient Hub after 3 notification attempts via text/email, your appointment letter will be posted to you.

When an appointment is booked or cancelled on the Trust’s patient administration system, you will receive a notification within the next 25 hours. Depending on whether you log in and accept your appointment, you may receive up to 2 further invitations to log in. These notifications are sent on consecutive days.

If you log in on receipt of the first notification, the system will stop sending you any further notifications, aside from 7 and 2 day reminder messages about your appointment.

You will be able to see all your appointments in Patient Hub. You can access each appointment individually within the portal.

Yes, the link to your account will stay the same, so you can go back to any text message or email. Every time you are notified about an appointment you will get a reminder of the link. If you want to go back into your Patient Hub, just use that link. Or, if you have deleted or lost the original text message, the link is above on this page.

Yes, you can click on a diary button to add a reminder to your smartphone or computer calendar.

Yes, the service is designed with security in mind. The text message or email regarding your appointment will be sent to you by the hospital. Once you click on the link, you will be asked for two forms of identity, to check it really is you. This will be either your mobile number, email address and your date of birth. This information, coupled with a 6-digit authentication code, forms the secure 2-factor authentication.
