We’d really like to hear from you.

If you've been a patient on one of our wards, been to one of our outpatient clinics, had a baby here recently or attended our Emergency Department (A&E) you can tell us what you think via the 'Friends & Family Test'.

It's our way of finding out if you'd be happy for your loved ones to be treated by us - or for you to tell us where we might need to improve.

Some of our patients want to tell us about their hospital stay or visit - so choose to do 'Friends & Family' online and tell us what you think. It's really easy to do - just ask for a form in the clinic area or fill in our online form.

You can also contact our PALS Team to leave a comment or compliment or tell us about any concerns or complaints.

If you've experienced health care recently, either as a patient yourself or as a carer or friend of someone else, please tell us how it was. What was good? What could have been better?

Your story will be handled confidentially by the non-profit service Care Opinion. Your story may appear (without your name) on their website. Care Opinion asks the health service to respond to the feedback you share, but does not reveal your identity.

This service is independent of Chesterfield Royal Hospital NHS FT and the NHS. For more information about Care Opinion and how your story is handled, visit www.careopinion.org.uk.
