It would really help us in planning your care if you could register with My Pregnancy Notes and fill out as much of the referral form as possible. 

Our maternity care is provided at the Chesterfield Birth Centre where we have midwifery-led and consultant-led delivery suites. 

It is important you take a pregnancy test to confirm you are pregnant. If you have a regular monthly menstrual cycle, a missed period is often the first sign you could be pregnant. This may not be obvious for women who do not have regular menstrual bleeds.

You must be registered with a local GP practice before you register your pregnancy with us via My Pregnancy Notes. If you’re not registered with a GP, you can find a GP in your area and contact them to register.
